AVFRST

Autonomous-vehicle incident management

A structured response when seconds matter

Operating AVs brings unique challenges in managing incidents effectively. AVFRST is designed to provide an incident-management framework built on ITIL principles — focused on prevention, prompt response, and continual improvement.

The framework

Built on ITIL, tailored to the operator

The framework can be tailored to the specific requirements of the operator and their operational domain, considering the regulatory environment and urban complexity. Its objectives: minimize the impact of incidents on public safety and traffic flow, ensure prompt response and stakeholder communication, identify root causes to prevent recurrence, and improve public trust in autonomous-vehicle technology.

1

Incident identification

Detected through vehicle monitoring systems, public and operator reporting, and integration with city traffic-management systems. Real-time identification is crucial for prompt response.

2

Incident logging

Logged into a centralized Incident Management System — time, GPS location, vehicle details and automation level, an initial assessment, and sensor data including speed, braking, and camera footage.

3

Categorization

Classified by severity and impact: minor (no injuries, minimal damage), moderate (minor injuries or moderate damage), or major (severe injuries, fatalities, or major disruption).

4

Prioritization

Ranked by severity and impact on public safety so resources are allocated efficiently, with the most severe incidents receiving immediate attention.

5

Investigation & diagnosis

Sensor data and logs analyzed for anomalies, coordination with manufacturers for technical assessment, and review of footage and conditions to identify root cause.

6

Resolution & recovery

Corrective action — recovery vehicles, emergency medical services, software or hardware fixes, and traffic-management measures to restore normal flow.

7

Closure

Formally closed once corrective actions are verified, the resolution and lessons learned are documented, and stakeholders are notified as appropriate.

Communications plan

Everyone informed, in the right order

Coordination between local DOT, emergency services, and the operator; public communication via traffic alerts and news outlets; and regular updates to manufacturers, officials, and regulatory bodies on resolution progress.

Continual improvement

Every incident makes the next response better

Root-cause analysis, post-incident reviews with stakeholders, and ongoing training for operators, responders, and city personnel on handling AV-related incidents effectively.

An end-to-end program, designed for your fleet

For operators who want it, AVFRST is designed to provide a 24x7x365 incident-management program drawing on experience across the transportation and software industries.
